PPO (Preferred Provider Organization) Dental Coverage
In-Network vs Out-of-Network:
In-Network: Typically, you pay your portions for the procedure according to the policy.
Out-of-Network: Out-of-Network benefits typically will be paid based on MAC fees. MAC means the Maximum Allowable Charge for your plan. You may be responsible for the difference between the MAC and the actual dental charge from a Non-Participating Provider.
NOTE: You should always check with your dentist about procedure and cost.
These are the features of the plans that make them distinct from each other. There may be limitations and exceptions to the plan. These may help you decide which type of dental plan is a better fit for your needs. Let’s compare these plans based on the following:
- Deductible: a specified amount of money that the insured must pay before an insurance company will pay a claim
- Coinsurance: a type of insurance in which the insured pays a share of the payment made against a claim.
- Annual maximum: Sometimes referred to as a plan maximum, or maximum amount – a dental annual maximum is a total your dental plan will pay toward your care during any one plan year.
- Waiting period: A waiting period is the amount of time an insured must wait before some or all of their coverage comes into effect.
- Typical Types of dental services:
- PREVENTIVE CARE Routine Exam Bitewing X-rays Cleaning
- BASIC CARE Full Mouth/Panoramic X-rays (1 in 3 years) Sealants (ages 6 through 16) Restorative Amalgams Simple Extractions
- MAJOR CARE Onlays, Oral Surgery, Implants, Crowns, Crown Repair, Endodontics (nonsurgical), Periodontics (nonsurgical), Periodontics (surgical), Denture Repair, Dentures Bridge, Complex Extractions, Anesthesia
Vision
Vision plans typically cover in-network providers and out-of-network providers. The cost is lower for using in-network providers. The typical coverages are exams, lenses, frames, contacts, and may give discounts to Laser Vision Correction.
