What is Medicare C-SNP?

Medicare C-SNP indicates Chronic-Condition Special Needs Plans for people with chronic or disabling conditions and is offered by independent insurance companies.

What qualifies you for C-SNP?

You qualify If you have a chronic or disabling condition such as, Diabetes, (COPD) chronic obstructive pulmonary disease, (ESRD) End-stage renal disease or congestive heart failure or cardiovascular disease. You must have Medicare Part A and Part B and live in the service area of the plan.

Is a C-SNP based on income?

NO, it has nothing to do with income. There are (LIS) Low Income Subsidies that are base on income and assets to help you with medical and insurance costs, which we can help you with.

What are the premiums for a C-SNP?

In most cases there is no premium, no doctors copays, no deductibles,  it should include your Part D drug plan with no deductible and may offer an additional tier for selected care drugs for your condition.

How can you find out if you qualify and if so, when can you apply?

You must have Medicare Part A and Part B and live in the C-SNP service area, and you must have been diagnosed by a doctor indicating that you have a chronic or disabling condition. You can apply for a C-SNP year round. If you have an Advantage Plan, Medicare Supplement Plan or just Medicare Part A and B, you can change to a C-SNP if there is one offered in your area.
